Leaving hospital or a rehabilitation centre


Discharge planning

Before being discharged from hospital or a rehabilitation facility, your medical team will prepare a report called a discharge plan.

   

The discharge plan records the treatment you have received and outlines the care and support you will likely need after you have been discharged.


The discharge report will be provided to TIO MAC to ensure we have a thorough understanding of your injuries and the medical and rehabilitation care you will require after leaving hospital. 

Taking charge of your recovery

By taking an active part in the discharge planning, you will learn about your injuries and future rehabilitation needs.  You will have a better understanding of what is necessary for you to return to health and get back to your highest level of functioning as soon as possible.


We encourage you to stay positive on all the things you can do and those you enjoy doing. Keep motivated as we work with your treatment providers to help you.

We will consult with your treating medical practitioner around the planning of your recovery and supporting your return to health.  We will ask your doctor to:


  • provide an initial assessment and clear diagnosis of your condition 
  • complete MAC medical and work capacity certificates when required 
  • offer recommendations and referrals for specialist treatment to help your recovery
  • regularly assess your progress towards regaining your employment capacity 
  • communicate directly with your treatment and rehabilitation providers

Attend appointments

When you leave hospital, you will be advised if you need to attend appointments made by the hospital through the outpatient’s department. 


It is important that you attend these appointments so that your medical team can provide ongoing care and re-assess the treatment necessary for your continued recovery. 

Hospital Occupational Therapist

If required, an occupational therapist (OT) will visit your home (or where you intend to live following discharge from the hospital or rehabilitation facility) to complete a home assessment. 


The occupational therapist will assess the suitability of your home and provide recommendations to MAC for any aids, equipment, care and support that you may need. They may also arrange short-term loan of equipment to help you make a safe transition from hospital.

Assessment of daily needs

If necessary, TIO MAC will arrange for an occupational therapist to assess your daily living needs in the first few weeks after you leave hospital and return home.


This assessment will review your functional performance including self-care and general domestic activities in and around the home, making recommendations for support, aids or equipment that will assist you in being independent in your home whilst recovering.

Ask questions

A good way to understand what is happening with your health is to ask your doctor questions.  This will help you take charge of your recovery.  You can make better decisions about what is right for you.

Your participation

Your active involvement means attending appointments and following treatment and exercise plans. If you are unable to attend an appointment, please let us know so that this can be rescheduled.
